A doctor's first question is ఏం జరిగింది? and everything you answer sits in the past. You already have both halves: తిన్నాను, తాగాను, వచ్చాను for what you did, and తినలేదు, తాగలేదు, రాలేదు for what you did not. Remember that the past negative never changes for person — నేను తినలేదు, ఆమె తినలేదు, వాళ్ళు తినలేదు are all the same word.
నొప్పి, జ్వరం — 'నాకు నొప్పి ఉంది.' 'నాకు జ్వరం వచ్చింది.' చోటు — 'ఆసుపత్రికి వెళ్ళాను.' 'వైద్యుడి దగ్గరకు వెళ్ళాను.'
Telugu does not say 'I have a pain' with a verb for 'have'. It uses the dative pattern you learnt with ఇష్టం and జ్వరం: నాకు నొప్పి ఉంది, నాకు జ్వరం వచ్చింది. For where you went, ఆసుపత్రి takes plain -కి, while a person takes the possessive form plus దగ్గర — వైద్యుడి దగ్గరకు వెళ్ళాను, literally 'I went to near the doctor'.
వైద్యుడు ఏం చెబుతాడు? — 'మందు తీసుకోవాలి.' 'ఎల్లుండి రండి.' 'పని చేయకండి.' మీరు ఏం చెబుతారు? — 'నేను పని చేయలేను.'
The rest of the visit is the mood system of this level, all in one place: obligation — మందు తీసుకోవాలి; a polite command — ఎల్లుండి రండి; a prohibition — పని చేయకండి; and your own inability — నేను పని చేయలేను. Keep చేయలేను (I am unable) apart from చేయను (I will not): the doctor is told the first, not the second.

Play both parts of the visit aloud. As the patient, say three sentences: one symptom with the dative pattern (నాకు నొప్పి ఉంది), one thing you did not do yesterday (నిన్న నేను పని చేయలేదు), and one thing you cannot do today (నేను వెళ్ళలేను). Then answer as the doctor with three instructions — మందు తీసుకోవాలి, ఎల్లుండి రండి, ఈరోజు బయటికి వెళ్ళకండి.
